Our Remote Heart Failure Management program is unique to Craig Cardiovascular Center. It enables daily home monitoring and direct correspondence with patients who need a higher level of care. A dedicated heart failure nurse reviews vital sign trends every day and intervenes the moment data suggests decompensation. Published outcomes from CCC's own JMIR study show a 93% reduction in heart failure ER visits, 83% reduction in heart failure hospitalizations, and 73% reduction in unscheduled office visits compared to standard care.
